Consent of Adult, Parent or Guardian and Publicity Release

In consideration of being allowed to participate in any of DEK Superstars programming and or related events or activities and or use of its facilities, I the undersigned, acknowledge, appreciate and agree that:

The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while these occurrences are rare and particular skills, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and

I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of DEK Superstars executives immediately; and

I am aware that DEK Superstars is an outdoor facility and all of its activities are played outdoors under various weather conditions, and I fully UNDERSTAND AND FREELY ASSUME all such risks involved with playing outdoors which include but are not limited to potential surface issues caused by inclement weather and that injury may result, including but not limited to by virtue of collisions with the surface, dasher boards, net and net structures and other structures on or near the playing area.

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS DEK SUPERSTARS LLC, AND NASSAU COUNTY POLICE ACTIVITY LEAUGE, (NCPAL),THE NCPAL MASSAPEQUA UNIT, THE NASSAU COUNTY POLICE DEPARTMENT, THE COUNTY OF NASSAU,USA Ball Hockey. and release from responsibility the said Corporations, its coaches, volunteers, employees, agents, officers, directors, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used for the activity (“Releasees”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. 

I the undersigned, being the parent or guardian do hereby grant permission for his / her participation in all activates, athletic or otherwise, sponsored by Nassau County Police Activity League (NCPAL) DEK Superstars LLC,  the Nassau County Police Department and the County of Nassau and release from responsibility the said Corporations, its coaches, volunteers, employees, agents, officers, directors, for any injury, loss of life or other loss or damage as a result of participation in any activity of the Nassau County Police Activity League and DEK Superstars. Furthermore, I fully understand the Nassau County Police Activity League and DEK Superstars sponsored events and in the event an emergency occurs, medical services and / or transportation will only be provided through the community’s emergency medical system.

Additionally, I hereby give the Nassau County Police Activity League, its affiliated unit and DEK Superstars permission to use my child’s name, likeness, image, voice and or appearance as such may be embodies in any pictures, photos, video recordings, audiotapes, digital images, and the like, taken or made on behalf of the NCPAL and DEK Superstars programs and activities. I agree that the Nassau County Police Activity League and DEK Superstars programs have complete ownership of such pictures, etc., including the entire copyright, and may use them for any purpose consistent with the NCPAL’s mission. These uses include, but are not limited to illustrations, bulletins, exhibitions, videotapes, reprints, reproductions, publications advertisements, and any promotional materials in any medium now known or later developed including the internet. I acknowledge that I will not receive any compensation, etc. for the use of such pictures, etc., and herby release the Nassau County Police Activity League, its affiliated units and DEK Superstars from any and all claims which arise out of or are in any way connected with such use. I give my consent to the NCPAL and DEK Superstars to use my child’s name and likeness to promote the NCPAL and DEK Superstars programs affiliated units, and / or their activities.

As the Parent or Guardian of a P.A.L. and DEK Superstars participant, I have read the above passages, understand them thoroughly and I will disclose the above stated policies to my child and any other spouse / spectator who may attend a P.A.L. and DEK Superstars activity.
